Police Special Needs Program
The Old Bridge Police Department has implemented the use of a Special Needs Information Form. The purpose of this form is to provide the department with emergency contact information, as well as specific characteristics with regard to individuals with special needs (i.e., Autism, Alzheimer's, non-verbal, etc.)
If you have a family member that you would like to enroll in this voluntary program, please complete the form below and submit with a photograph. The form will remain on file with on file with the Police Department and the information provided to the department will be kept confidential. You are not obligated to disclose medical information.
Please return the completed form via email to mjeffries@oldbridge.com or drop off in person at the P.D., or mail to Old Bridge Police Department, One Old Bridge Plaza, Old Bridge, NJ 08857.
